Chronic illness accounts for the bulk of health spending across Medicaid, Medicare and other payers. And chronic disease tends to be more prevalent in those rural, suburban and urban communities with fewer health professionals and greater socio-economic challenges. Our speakers share how strong primary care is delivering better outcomes at a lower cost at health centers and practices in these communities – and identify policies to build on that success.
