With rising health costs burdening federal programs and the private sector alike, America needs new strategies to curb health costs while improving health care. The National Coalition on Health Care  Forum series is dedicated to showcasing health care innovators and thought leaders who can help the nation meet this challenge.At NCHC’s October Forum, we will hear from a doctor, a nurse, and a pension fund representative who have charted paths toward better care and better health at much lower costs.Ms. Frizzera  operates an innovative program of care coordination for the frailest nursing home patients, designed to improve care and reduce unnecessary hospitalizations.Mr. Estabrook has implemented and participated in the Diabetes Prevention Program (DPP), which he selected as a benefit for the employees of the Ohio Police and Fire Pension Fund in Columbus, OH.  The DPP is a community-based health education program that, according to recent studies, can lower the rate of diabetes onset by 34% over ten years.Dr. Penso, a family physician, has leveraged alignment of consumer and provider incentives to lower cost and improve quality in both his current role and past work as Medical Director with the integrated delivery system Sharp Healthcare in San Diego.With the nation’s health care challenges taking center stage in the national debate over deficits and debt, there’s no better time to hear from the innovators who have found and implemented real world solutions that lower costs and improve quality.
